All of our forms are in PDF format. 

 

If you should need a different format (i.e., Word, Word Perfect, etc.), please CONTACT US.  We will be glad to convert the forms and documents to a different format for you!

 

If you are not able to download and print out the forms, please be sure to let us know and we'll be glad to e-mail, fax or mail the forms to you.

If you would like to complete the forms ELECTRONICALLY, we recommend using Adobe Acrobat Reader DC software that is available for FREE directly through the Adobe website:  https://get.adobe.com/reader  or click on the icon below.  The software will be installed on your computer.  Simply download our forms, open with Adobe Reader and complete.  When you are done you can save them to a file on your computer so that you will have your own copy and then you can send copies to us by attaching them to an email to us.  All without ever printing out a form!  (Save time, ink, paper, etc.!)

 

 

 

There is also another great FREE online software program called PDF FILLER.  This is an ONLINE version, so you do not need to download the software to your computer.   You can download the forms from our website to your computer and then import them into this or you can simply add the url of the document that you wish to complete.  It is also FREE and easy to use.  Their website is:  https://pdffiller.com.  Just click on the "Drag and Drop Documents Here to Get Started" in the middle of the page and follow the prompts. 

 

We have tried and used both of these without any problems.  Both of these are GREAT because you do not need to print out any forms, complete and sign by hand and/or scan anything back into your computer.  Save a lot of time and work and you can use ANY pdf document.

A PRELIMINARY ASSESSMENT FORM must be completed, submitted and approved for each dog PRIOR TO scheduling services.

 

1.)  Client & Household Profile (please provide your basic contact information)

2.)  Pet Profile (one for each pet/pet enclosure) 

3.) Veterinarian Treatment Authorization/Medical Records Release

4.)  Pet Care Service Agreement

5.)  Medication Administration Permission form

6.)  Emergency Credit Card Authorization form (for emergency medical care ONLY and required for ALL clients, even if you opt-out.)

7.)  Pet Guardianship Assignment (Optional, but recommended)
